Keeping Medication Cold Without Power

Most blackout advice worries about the fridge full of food. The genuinely serious problem is the small box in the door: insulin, some antibiotics, eye drops, certain injectables, and a handful of other medications that need to stay cold.
Food spoiling is expensive. Medication going out of range is a health event, and it is the failure that most reliably turns an inconvenient outage into a real one.
The good news is that a cool chain is easy to improvise, and the temperature windows are more forgiving than most people assume — as long as you know them rather than guessing.
The 10-minute version
- Cool, not frozen. Refrigerated medications generally want 2–8°C. Freezing ruins many of them, so ice must never touch the medication directly.
- Insulin has real tolerance. Unopened insulin belongs in the fridge at 2–8°C, but it can generally be kept below about 30°C for up to 28 days — which covers almost any blackout. Check your own product and pharmacist.
- Ring your pharmacist, early. They answer this question constantly, know your specific medication, and can arrange an emergency supply. Do it while the phone still has battery.
The esky method
This takes ten minutes and holds temperature for a day or more.
What you need: an esky or insulated bag, frozen water bottles or refreezable gel packs, a towel or tea towel, and a thermometer.
- Freeze water bottles now, not when the power goes. Bottles in the freezer are the single most useful preparation on this page, and they double as drinking water and as a way to keep the freezer cold longer.
- Put the frozen bottles in the bottom of the esky.
- Lay a folded towel over the ice. This is the critical step — it stops the medication touching anything freezing. Direct contact with ice is what ruins insulin.
- Put the medication on the towel, in its original packaging.
- Put a thermometer in with it. A $6 fridge thermometer, or any digital probe. Guessing is how medication gets frozen.
- Close it and leave it closed. Every opening costs hours.
- Check the temperature twice a day and swap the bottles when they thaw.
Aim for 2–8°C. If it is reading below 2°C, add another towel between ice and medication or remove a bottle.

Do not generalise from one medication to another — the windows differ, and your pharmacist has the specific answer. As reference points:

The pattern is worth internalising: freezing is usually worse than warm, and “warm for a while” is often acceptable when “frozen briefly” is not.
Before the outage
Ring the pharmacy and ask two questions about each refrigerated medication you take: what is the temperature range, and how long can it be out of the fridge? Write the answers on a card and put it with your emergency kit. Ten minutes, once, and it removes all the guesswork on the day.
Freeze water bottles. Ongoing, free, and useful three ways.
Buy a cheap thermometer. Six dollars ends every argument.
Keep a week of all medication in the go-bag, in original packaging, with a written list of drugs and doses.
If anyone is power-dependent for a device — CPAP, oxygen concentrator, home dialysis, feeding pump — register as a life support customer with your energy retailer. One call and a doctor’s form. During and after
Write down when the power went off. Every judgement afterwards depends on it.
Do not open the esky to check the medication. Check the thermometer instead.
If it has gone out of range, do not guess and do not throw it out unprompted either. Ring the pharmacist and describe what happened: how long, how warm or cold, and what the medication is. Some medications tolerate an excursion; some do not.
If it froze, assume insulin is ruined and speak to the pharmacist before using it.
